This card offers a convenient way for keeping you,
your employees, family members and friends healthy
during the flu season. Here's how it works:

Each card can be redeemed for a pre-paid flu
shot vaccination at any of the 7,500 Walgreens
pharmacies or 350 Take Care Clinics nationwide.

Flu vaccinations are available in EVERY
store.... EVERY day throughout the flu season
during regular pharmacy hours .... without an

this year's seasonal vaccine. You will only need

Purchase of this card constitutes acceptance of these terms:
This card can be redeemed only for administration of (1)
Seasonal Flu Vaccination or (1) Preservative-Free Seasonal
Flu Vaccination. This card may not be redeemed for inhaled
seasonal flu immunizations or other types of vaccinations.
Gift card may be redeemed at any Walgreens Pharmacy in the
U.S. or Puerto Rico, while a vaccinating health care
provider is on duty. State, age and health related
restrictions may apply to vaccine administration. Cards are
not valid at Duane Reade locations. The card is loaded with
$29.99 in value. If the cash price of a seasonal flu shot is
greater than $29.99, customer will be responsible for the
difference in price. If the cash price of a seasonal flu
shot is lower than $29.99, the card holder will be entitled
to a credit on the card equal to the difference in price for
use in a subsequent year. Card may not be used in
conjunction with any co-insurance, third party payors or
discount cards or offers. This card may not be used to
purchase any other items. No refunds or returns will be
issued on gift cards. Card is not reloadable and no change
will be given except as and where required by law. Card can
be redeemed at any Walgreens Pharmacy or Take Care Clinic.
The card will expire five (5) years from the date of card
activation except where prohibited by law. Lost or stolen
cards may not be replaced.
Based on manufacturer availability, seasonal flu shots are
generally available for administration between September and
January in any given calendar year. If the supply of vaccine
is depleted at the time of gift card presentation, gift card
will be honored during the following flu season up to five
years from the date of initial activation.
Patient care services at Take Care Clinics are provided by
Take Care Health Services, an independently owned
professional corporation whose licensed healthcare
professionals are not employed by or agents of Walgreen Co
or its subsidiaries, including Take Care Health Systems,
LLC.
